    OSHA RECORDKEEPING FORMS

    Notice:
    In the December 17, 2002 Federal register (67 FR 77165-77170) OSHA announced its decision to modify the OSHA Form 300 to include an occupational hearing loss column. Employers required to complete the injury and illness forms must begin to use the modified forms on January 1, 2004. You can access these forms by using the link below which refers to Calendar Year (CY) 2004 and beyond. If you need copies of the forms used for CY 2002 and CY 2003, you can access those forms using the link referring to those years.

    This will take you to the "DOWNLOADING AND INSTALLING ACROBAT READER" page. Read the included instructions and click on the "DOWNLOAD VIA FTP" hyperlink.

    Go have a cup of coffee, this should take 5-45 minutes depending on the speed of your connection.

    Once downloaded, follow the installation procedures included with the Acrobat Reader. You will then be able to read and print the pdf files included in this web site.
